[00:00:56] Before I go on to the episode, what is new? What is greater in life? I'll tell you what, there's a lot of moving parts.

[00:01:03] I have some exciting news coming down the line with sports performance on the CG Academy.

[00:01:09] We're right on to the CashPTA clinical elevation course. I think we're in module four.

[00:01:15] We're talking about why cash pay patients drop off.

[00:01:20] We just went through, gosh, three series on the initial valuation, seven point cash pay movement screen, and going through that whole scenario.

[00:01:29] So if you ask me, you know, what my passion is, it is cash pay patients and helping sports PTs, cash PTs really dive into a sector that it will be a necessity when we talk about what the future of our profession is.

[00:01:46] And that cash PTs clinical elevation course is everything that I believe in and wholeheartedly feel at the moment.

[00:01:53] So we have progress on that. Everything's going great. My team at Sports Performance, we're growing.

[00:01:59] We've got some new things coming up for annual planning, which is fantastic. We have all those moving parts.

[00:02:03] My family.

[00:02:06] My son, Zach, is absolutely crushing flag football.

[00:02:10] I haven't seen him this excited.

[00:02:12] And here's a fun one.

[00:02:14] As a parent, you see your kid.

[00:02:17] You believe in your kid.

[00:02:18] And when other parents tell you confirmation of things, it's nice knowing that you're in the right direction and you believe what you see.

[00:02:25] But this Saturday, you know, Zach has been struggling with playing soccer on a regular basis.

[00:02:31] He enjoys it.

[00:02:32] But man, for some reason, like flag football is his thing.

[00:02:35] So this Saturday, a parent, he was playing great.

[00:02:38] He was actually like enthusiastic.

[00:02:39] He was participating.

[00:02:41] And a parent said, Zach is really coming around and something to that effect.

[00:02:47] And another coach said, like, what did you eat today?

[00:02:50] So basically, you know, I saw my son participating more.

[00:02:54] And I think one of the things, you know, I asked him, I said, why did you try so hard on this one?

[00:02:58] Like, what is it?

[00:03:00] Here was his answer.

[00:03:01] He says, dad, we were losing by a lot and I was getting tired of it.

[00:03:06] So I think they were down like, I don't know, 6-0.

[00:03:08] And he just got frustrated.

[00:03:11] And he actually like put in effort.

[00:03:13] So I always say that, you know, people, and this goes into today's episode, but people don't change unless they're fearful or frustrated with something.

[00:03:24] So, for instance, you know, Zach was frustrated with losing that bad.

[00:03:29] So he made that choice.

[00:03:29] So he had to be able to go to the other side.

[00:03:31] So good for him.

[00:03:32] I'm really proud of him.

[00:03:34] And I'm like, I'm every stage is a fun stage with kids.

[00:03:39] But Zach at the eight years old is a very fun stage.

[00:03:43] Like as a dad, I've been waiting for this stage.

[00:03:45] Like I've loved every stage until now.

[00:03:47] But man, I am like loving it because he's asking better questions about, you know, dad, why these things happen.

[00:03:54] He's immersed in sports.

[00:03:56] I just, I see him growing his personality and that's just, it's amazing to watch as a dad.

[00:04:01] So my other two kids are great.

[00:04:03] Jacob is, he is bossier than ever.

[00:04:06] Man, if I had one kid who is the dictator in the family, that would be my son, Jacob.

[00:04:11] He just tells you like it is, no emotion, you know, like that was not nice dad or feed me now.

[00:04:18] He doesn't really ask.

[00:04:19] He just tells you what to do.

[00:04:20] And then my beautiful daughter, she's doing amazing with gymnastics.

[00:04:23] So those are all the things that are going great in my life.

[00:04:26] And just truly blessed for it all.

[00:04:29] And wherever you are in your personal life, in your professional life as a sports PT or cash PT,

[00:04:35] I think I heard this recently and I think it's very important, especially with like,

[00:04:41] by the time you listen to this, it's, you know, election week and there's things moving all over the place

[00:04:46] with economics, healthcare, personal lives, all these other things.

[00:04:49] And I think that the more grounded you are, the more you are in like your local economy

[00:04:53] and just like rely on your family, your local small circle, life is pretty amazing.

[00:04:59] And I don't think that, you know, a lot of the things external to that really should influence you that much.

[00:05:04] So I think that you enjoy, you know, your family, your small circle of friends,

[00:05:08] maybe your work environment, your patients.

[00:05:09] I think you just get fulfilled by that.

[00:05:11] And I'm in a zone right now where it's very hard to touch that inner circle from an emotional standpoint.

[00:05:18] I'm very, very consistent, very, very blessed to have all that.

[00:05:23] But I also, it's something that's been created.

[00:05:25] So for a lot of you who feel like there's just a lot going on in your life,

[00:05:28] one of the things I can highly recommend is just staying in a small circle where you can control

[00:05:33] and not letting anything else come in.

[00:05:35] So that's my small tidbit.

[00:05:37] And that leads me right into the podcast.

[00:05:39] So the thought process behind this podcast was why, you know, why do people make the decisions that they do?

[00:05:48] And, you know, how can sports physical therapists, cash pay physical therapists,

[00:05:52] how can you make a larger impact beyond just pain management?

[00:05:56] And I'm not even talking about like wellness programs.

[00:05:58] I'm not talking about strength programs.

[00:06:00] I'm just talking about how do you help the person in front of you because that's probably why you got into this profession.

[00:06:05] So how do you help the recovery process and the psychology behind that?

[00:06:12] And so let me, let me dive into this, right?

[00:06:15] If, if you were a personal trainer and you had a client come to you and say,

[00:06:21] I want to come to the gym five times a week and I need help.

[00:06:26] That's fantastic.

[00:06:27] We can help you get there.

[00:06:29] How many times were you going to the gym before?

[00:06:31] One time.

[00:06:32] Okay.

[00:06:33] So you want to make this leap from one to five.

[00:06:35] Well, the reality is if you really understand the psychology of decision-making,

[00:06:41] something big had to happen in their life for me to make that drastic of a change.

[00:06:46] And once you understand that, you can better help that customer or that patient or that client

[00:06:52] because it's not about weight loss.

[00:06:55] It's not about going to the gym five days a week.

[00:06:58] It's the better question is why did you make this leap from one to five days a week?

[00:07:03] And some people don't feel comfortable asking that question or they don't think it's relevant.

[00:07:07] It's highly relevant because what they're asking for is your service.

[00:07:10] They're asking for your help to get to five days.

[00:07:13] The question is why choose five days?

[00:07:15] Why not three?

[00:07:18] The better thing you can do is ask the better questions so you can get to the root faster.

[00:07:23] And this is about the psychology of why they make the decision to recover,

[00:07:27] the decision to go to the gym, the decision to save more, the decision to lose more,

[00:07:31] all these things.

[00:07:32] So the question becomes why did you choose five days?

[00:07:34] Well, I thought that would be consistent.

[00:07:36] Great.

[00:07:37] Consistent for what?

[00:07:41] And they'll probably come up with a root which might be I want to lose weight.

[00:07:46] Great.

[00:07:46] We can't end it there.

[00:07:48] So you want to go from one to five days.

[00:07:49] Great.

[00:07:49] To lose weight.

[00:07:50] How much weight?

[00:07:53] 10 pounds.

[00:07:54] How did you choose 10 pounds?

[00:07:56] And they're going to eventually come to, I don't know.

[00:07:58] I just don't feel good about myself.

[00:08:00] Wow.

[00:08:01] Now let's have a conversation about that because now I can help you.

[00:08:03] But now we have to make long-term, short-term and mid-term goals in order to get you there.

[00:08:08] Well, great.

[00:08:08] Can you give me an idea of what it would take to help you feel good about yourself?

[00:08:11] Like were you starting with five pounds?

[00:08:14] 10 pounds.

[00:08:14] Do you want to fit in a certain pair of jeans?

[00:08:15] You have to get objective.

[00:08:17] And now let's take that back to a patient, an athlete.

[00:08:23] And someone says, I want to fix my knee pain.

[00:08:27] Knee pain, great.

[00:08:28] What is it stopping you from doing?

[00:08:30] Going up and down stairs.

[00:08:31] So the insurance-based PT would say, well, perfect.

[00:08:35] So that's a goal.

[00:08:37] Must be able to ascend and descend stairs without assist and 0 out of 10 knee pain 10 times in a day.

[00:08:46] That still doesn't solve why they came to you.

[00:08:50] They came to you because they were having knee pain and it's interfering with their stairs.

[00:08:55] Okay, why did you choose stairs out of all things?

[00:08:58] What's limited?

[00:08:59] Like what's so important about going up and down stairs?

[00:09:01] Oh, I have one of my grandma lives downstairs with us and I take care of them.

[00:09:08] So I want to be able to give back to them.

[00:09:10] I want to make sure that they're safe, but I have to be able to go up and down stairs.

[00:09:13] I live on the second floor.

[00:09:14] They live on the first floor.

[00:09:15] Oh, I see.

[00:09:16] So my goal here is to be able to help you with the, not the knee pain, but it's being able to help you care for your grandmother.

[00:09:23] That's a different conversation.

[00:09:25] Knee pain is just a segue into how you do business with them.

[00:09:29] How you help them.

[00:09:30] But the problem is that insurance has set you up to be focused so heavily on knee pain because that's how you got reimbursed.

[00:09:37] So as a sports PT, your job is, yes, fix the knee pain.

[00:09:42] Do whatever insurance says.

[00:09:45] But the right thing to do here is help the person with whatever they need.

[00:09:49] And the problem is, is that there's a psychological concept.

[00:09:53] There's a psychological challenge that made them ask for help to help them with their recovery.

[00:09:58] Now, you as a physical therapist need to understand that process to support and maximize the recovery.

[00:10:05] They might be saying knee pain, but what they really mean is caring for their grandmother.

[00:10:09] And if everything you focus on is knee pain, you're not maximizing what they can do.

[00:10:13] So the reality and the root is the psychological decision to come to a physical therapist had to come with a fear.

[00:10:20] You might have heard me in previous episodes talk about people make decisions on fear.

[00:10:28] The decision to go to the gym is based on the concept of, I don't look good in the mirror.

[00:10:35] Now, there's many decisions in their brain that's going through that process.

[00:10:40] Is it that they don't feel good about themselves and they'll never meet a partner?

[00:10:44] Is it that they're not going to look good for their wedding?

[00:10:47] Or is it that they're going to die sooner and they don't want that to happen?

[00:10:52] So there's several reasons why somebody doesn't feel good about themselves.

[00:10:55] The question is, what is the challenge for them?

[00:10:58] What's the fear?

[00:10:59] What's the root driver that made somebody come in and say, I don't feel good about myself.

[00:11:03] And I want to make an impact so I can feel better about myself so that I can, so that I can, so that I can.

[00:11:13] You have to be able to solve that question.

[00:11:15] That, that, the ending of that sentence is so critical to their recovery and to your results and to your reputation.

[00:11:22] So if you limit it to go up and down stairs, great.

[00:11:26] Add that question so that you can, so that you can care for your grandmother.

[00:11:34] That is a psychology behind decision-making.

[00:11:37] So the question is, what are your patients fearing?

[00:11:42] What made them go see you?

[00:11:46] What made them come see you four times a week?

[00:11:51] Why did they go through, why did they go through the ACL surgery at the age of 35?

[00:11:58] Why did they decide to get their Achilles repaired in their 30s?

[00:12:04] Why did they have the rotator cuff repair?

[00:12:08] What is it that made that decision?

[00:12:10] Because if you understand fear, you understand their deep concern.

[00:12:15] And that's the psychology of making that decision.

[00:12:17] So the question is, well, how do you find that out?

[00:12:20] That's the bigger question.

[00:12:21] Most physical therapists are afraid to ask that question.

[00:12:25] They think it's a little too, um, little too much because insurance has made it superficial.

[00:12:30] Uh, where I talk about pain.

[00:12:32] I only talk about pain in this region.

[00:12:34] I could talk about life, but I don't have enough time.

[00:12:37] I'm managing five other patients.

[00:12:39] But the real root is as you, today's modern era patient is Googling, they're YouTubing,

[00:12:45] they're on AI, they're figuring out their own options and how they can manage their own care.

[00:12:49] But the reality is they do not have the ability to solve their own problem.

[00:12:54] The question is, what is the thing that forced them to look for an answer to their current problem?

[00:13:00] And that's what you don't know and you have to.

[00:13:03] If you don't understand that, you don't understand the psychology of why they're there in front of you.

[00:13:08] So that psychological decision making is the thing you have to overcome.

[00:13:11] Um, so you might think, well, why can't I ask?

[00:13:16] Um, what is it bothering you with?

[00:13:18] Great.

[00:13:19] And then, uh, so that you can, what?

[00:13:22] The patient's going to be like that.

[00:13:24] Wait, what do you mean?

[00:13:25] It has to make sense in your head.

[00:13:28] Okay.

[00:13:29] Mrs. Johnson, I understand it's bothering you with knee pain, but you've probably had knee

[00:13:34] pain or knee discomfort before.

[00:13:36] And you were able to go up and down stairs with a little bit of discomfort.

[00:13:40] What was the final decision that made you come in today?

[00:13:43] What was the last straw?

[00:13:44] Would you say, I'm sick and tired of this.

[00:13:46] What made you come in or what made you go to your primary care?

[00:13:49] What made you reach out to your insurance?

[00:13:51] You have to dive deep and there's numerous ways you can do that.

[00:13:54] Numerous questions.

[00:13:55] But the reality is, even if I give you the best template, the best script,

[00:14:00] if you don't believe that you're doing this for the right reason,

[00:14:03] they won't give you the answer.

[00:14:05] Why?

[00:14:05] Because you're just reading off of scripts.

[00:14:07] Why didn't this work?

[00:14:08] Uh, so that you can, uh, what was the last thing that made you like,

[00:14:11] they don't feel that.

[00:14:12] And the reality is all you're trying to do is help them understand the best,

[00:14:17] the best way I can help you is understanding why you made the decision.

[00:14:22] What in your life was being interfered with.

[00:14:24] So that way you can, uh, be able to do things without discomfort.

[00:14:28] For some reason, this back pain, this knee pain, this shoulder pain was interfering

[00:14:33] with your life so much.

[00:14:34] So what's the thing, the one thing you want to be able to do once you're free from this

[00:14:39] pain.

[00:14:40] And once I understand that I can help you get there, but I can look at your movement.

[00:14:44] I can look at your strength.

[00:14:46] I can look at all these factors.

[00:14:48] None of those things are important as, as important as what's the thing you want to

[00:14:53] be able to accomplish.

[00:14:54] And then once they say, I'm concerned, I won't be able to care for my grandmother and

[00:15:00] she's going to slip and fall and have a broken hip.

[00:15:03] I see that psychology of why they made the decision is so critical to you helping them

[00:15:10] with that recovery process.

[00:15:12] Why?

[00:15:13] Because you get them.

[00:15:16] You are the only person that has understood that because that deep, uh, thought has been

[00:15:23] marinating in their head and no one else has gotten there.

[00:15:26] And that thing has been thought about at night at 9 PM, 12 PM throughout the day, text messages,

[00:15:33] journaling.

[00:15:34] No one has been able to get to that.

[00:15:36] And once you understand that you can help them faster because now your intent is pure.

[00:15:40] All right, Mrs. Johnson, let's go up and down.

[00:15:43] We're going to start with this stretch.

[00:15:44] We're going to start with this knee thing.

[00:15:45] Just remember, it's your grandma, it's your grandma, it's your grandma.

[00:15:48] Let's go through it.

[00:15:48] Let's go.

[00:15:49] And you now, you're not a cheerleader.

[00:15:50] You're not a motivator.

[00:15:51] You will get what they need.

[00:15:53] That psychological aspect is so critical because you can keep them high because you're the

[00:15:58] only one who's navigated why they're acting this way, what the real result is that they

[00:16:02] want.

[00:16:03] And once you conquer that, that celebration is like tears.

[00:16:08] Can you imagine?

[00:16:08] Like, gosh, I would, I would get emotional just thinking about that because my job is nothing

[00:16:14] more than helping people function at a higher level, to be more active, to live a better

[00:16:18] lifestyle.

[00:16:19] But I can't get to that lifestyle if I don't understand what I'm doing.

[00:16:23] And that's your job.

[00:16:25] So the next time you're stuck and you get monotonous and you're getting the same knee pain and range

[00:16:31] emotion and everything else.

[00:16:33] And you're absolutely sick and fed up with doing the same thing day in, day out.

[00:16:38] Realize your job is bigger, but you have to understand what you're striving for.

[00:16:43] Your job here is underline the psychological motivator to come to see you, not the external

[00:16:50] knee pain or the patella or the calf or the dorsiflexion.

[00:16:55] It's all irrelevant because they had that before.

[00:16:59] They had that before they came to see you.

[00:17:03] So why didn't that limited calf dorsiflexion, weak hip a year ago come see you?

[00:17:10] What happened?

[00:17:12] It magically went from a five out of five to a four out of five to a four plus to a four

[00:17:17] minus.

[00:17:19] What happened?

[00:17:21] Life happened to them.

[00:17:23] And if you can't find out what that life is, your recovery is average.

[00:17:27] And that's probably why you have people who don't come to see you.

[00:17:29] And usually the case is they leave by visits.

[00:17:33] If you're in cash, visits three to five and your insurance six to eight because you were

[00:17:40] a waste of time.

[00:17:41] There was no more value you were bringing to them.

[00:17:44] In order to streamline the recovery process, understand the psychological aspects before that.

[00:17:49] So ask better questions.

[00:17:51] And it's everything that I've done in my cash pay practice.

[00:17:55] But also I learned this from doing this in pro sports.

[00:18:01] It was a time where I wasn't getting the outcomes that I wanted.

[00:18:05] And I had taken, gosh, by three or four years in, I had taken over like 20 CEU courses.

[00:18:10] I was just pumping them out.

[00:18:12] I had finished residency.

[00:18:13] I was doing like 20 courses a year.

[00:18:16] I was putting all my money into becoming the best clinician possible.

[00:18:19] And I wasn't getting the results.

[00:18:20] And I was like, I just need more mentorship.

[00:18:23] And finally, it wasn't one thing.

[00:18:25] I was just frustrated.

[00:18:27] And I was talking to coaches, strength coaches.

[00:18:29] I'm like, why are my athletes not getting better?

[00:18:31] And the reality was, I just wasn't getting them.

[00:18:34] Like, yes, they wanted to make more money.

[00:18:36] These are pro athletes.

[00:18:37] They wanted to make more money.

[00:18:38] So if I kept focusing on money, that wasn't the root.

[00:18:40] The root was feeding the family.

[00:18:43] It was the byproduct of the more money.

[00:18:45] So they were concerned that they stretched this calf or they stretched the hamstring.

[00:18:49] And they weren't going to be playing this weekend.

[00:18:52] And they weren't going to have the money they needed to feed their family.

[00:18:55] That's very different than aspiring to make more money.

[00:18:58] This athlete, okay, let's get your hamstring so we can.

[00:19:01] Get you $100,000 this weekend.

[00:19:03] That didn't resonate with them.

[00:19:04] So the psychological challenge of why they're going through recovery process,

[00:19:07] they were concerned about something.

[00:19:09] And that's why they sought my services.

[00:19:11] So the question is, what are your athletes and your patients concerned about

[00:19:14] that make them come to see you?

[00:19:17] That's a deep question you have to ask.

[00:19:19] And you've got to get it phenomenal in 30 seconds, 1 minute, 5 minutes of seeing them

[00:19:24] and getting to the root fast.

[00:19:26] And that just comes from good people skills,

[00:19:29] your ability to build those relationships quickly.

[00:19:31] Now, if you don't get this, your clinical skills,

[00:19:34] which you've paid a lot of money for, years, 7, 8, 9, 10 years of education,

[00:19:39] plus $1,000, $2,000 in annual CEUs,

[00:19:43] all of that is irrelevant.

[00:19:45] And that's so frustrating for a physical therapist to acknowledge.

[00:19:48] Wait, you're telling me that everything I've learned

[00:19:52] doesn't really matter unless I know what the patient wants.

[00:19:56] In insurance, those clinical skills are perfect.

[00:20:01] Anytime you want to have long-term outcomes for more than three months, six months, nine months,

[00:20:06] or a year, a lifelong relationship with the patient,

[00:20:09] you're going to have to go beyond just being a clinical specialist.

[00:20:13] You're going to have to go beyond understanding what a healthcare specialist is

[00:20:17] or a healthcare provider.

[00:20:18] And understanding if you want to help people, it's beyond that.

[00:20:21] For those of you who know, I got into physical therapy because my grandmother,

[00:20:26] she had a hip replacement.

[00:20:27] And we didn't have money and we didn't have health insurance.

[00:20:30] And so this was done by the county or city or whatever it was.

[00:20:34] And we didn't have any physical therapy.

[00:20:37] So I had to get on a bus.

[00:20:39] I didn't have a fax machine.

[00:20:40] So I had to get on a bus and get some printouts from a local physical therapy company.

[00:20:44] And I was 13 years old at the moment.

[00:20:45] And I realized that going through that process, I gave my grandma the life and quality

[00:20:53] that she really essentially created for me, which was to teach me how to walk,

[00:20:59] just be able to give me the quality of life that I had at the time.

[00:21:02] So for me, full circle was she was losing her independence.

[00:21:06] She was losing her ability to care for her grandchild.

[00:21:08] That was the root.

[00:21:10] That was the fear.

[00:21:10] And I get it now, but I didn't get it back then.

[00:21:13] I just thought like, oh, we're gonna get you walking again.

[00:21:15] But I realized the reason why she was so concerned is because she couldn't feel she can fulfill

[00:21:20] her responsibilities as a grandmother, whatever it is around the house, at work, all those things.

[00:21:26] And once I realized why people make decisions, it helps me get to the root so I can help them

[00:21:31] faster.

[00:21:31] And if you think that working on knee joints and back and shoulders is the root, it's only

[00:21:36] because you've only known insurance.

[00:21:38] And once you go to pro athletes, once you go to higher caliber people who are more motivated,

[00:21:45] more educated on how, why they want to get out of pain, what they're looking for, so frustrated

[00:21:51] with that model, then you will have to find another resource.

[00:21:56] And this is that resource.

[00:21:57] So if you are sick and fed up with that traditional model, you're like, people aren't listening

[00:22:01] to me.

[00:22:01] They're not motivated.

[00:22:02] It's because you're using the same framework and you have to be able to adapt and get deeper

[00:22:08] on a psychological level.

[00:22:09] And that is what really makes a difference on a great physical therapist, a phenomenal

[00:22:13] physical therapist.

[00:22:14] And an average physical therapist who just sees the same things day in, day out, hates

[00:22:19] their job, five years of boredom, burnout.

[00:22:21] And they just go through this repetitive cycle and they only grow because of your experience,

[00:22:25] not because of outcomes.
