[00:01:20] Welcome back to the podcast.

[00:01:22] Welcome all new listeners and welcome back to those who have heard my content in the past.

[00:01:27] Before I go on to today's episode,

[00:01:29] I want to share what's new, what's the latest in greatest?

[00:01:34] Well, today my son, I feel all the listeners out there.

[00:01:38] My son Zach has been pushing my buttons.

[00:01:41] Today he told me, ready for this?

[00:01:43] I went to give him a hug and a kiss right before he went with his friends.

[00:01:47] And as I approached him, he pushed my hand away and pushed me away.

[00:01:51] And he says, you just missed your opportunity, Dad.

[00:01:54] This is coming from an eight-year-old.

[00:01:55] I said, what did you learn that?

[00:01:57] And my wife said, you know what Chris?

[00:01:58] You tell him that every night if he doesn't behave properly,

[00:02:01] or if he asks for dessert a little bit late, we tell him, hey, you must try opportunities to late.

[00:02:06] So I got punched in the face today by my son with that.

[00:02:09] And so that hurt and stomach hurts will deep.

[00:02:12] My daughter's doing great.

[00:02:14] She just had a current agemnastics competition coming up.

[00:02:18] So I'm excited for her.

[00:02:20] And my son Jacob is doing great.

[00:02:21] We are doing a Disney cruise coming up.

[00:02:25] And he is 90% party-train.

[00:02:27] And so all good things, life is great.

[00:02:30] Backers doing stable, steady.

[00:02:32] That's all I want in life.

[00:02:34] Just keep it at bay.

[00:02:35] I'm a wife and I are super excited about this cruise.

[00:02:37] Just time away, decompress, and just have a great time with the kids.

[00:02:42] So on to today's episode.

[00:02:45] Why are we talking about if cash-based physical therapies for everyone?

[00:02:48] Well, it's not for everyone.

[00:02:50] And I've mentored a lot of people who struggled with this.

[00:02:53] You know, they will consult with me, they'll ask me, you know,

[00:02:56] it is where, what direction am I going?

[00:02:59] And in the end, when I take a step back as a mentor.

[00:03:02] My job is to help people see a clearer road, a clearer path.

[00:03:07] And after having discussions, I realized there are six qualities that I would say that

[00:03:12] you know, really successful physical therapists have and not everybody has them.

[00:03:17] And so for those of you who are struggling, if you're a cash-based physical therapist

[00:03:20] and if you're struggling, I would try and hone in on some of these things.

[00:03:24] And if you're considering cash-based physical therapy, I would see if you qualify for this checklist.

[00:03:29] Do you have all these things and are you going to be successful?

[00:03:33] And most people think, well, I'm good at my clinical skill.

[00:03:38] I don't understand why it can't be good in cash-based physical therapy.

[00:03:41] So before you do that, the sick quality is I'm going to cover today.

[00:03:44] Have nothing to do with skills.

[00:03:47] And because those cause always be trained, anybody can be trained on the clinical skill physical therapy.

[00:03:52] But it's the intangibles and everything else outside of that.

[00:03:55] So if you were interested in physical therapy, this is going to be a good qualification checklist.

[00:03:59] If you're in the thick of it and you're wondering what else you need to hone in personally,

[00:04:04] like what are you not seeing?

[00:04:05] These are going to be fantastic for you.

[00:04:06] And if you weren't successful on cash, this may be some of the reasons.

[00:04:11] So we'll go through that and I think it's very important for you to understand,

[00:04:14] you know, what it is that you're missing, what is it you're not seeing.

[00:04:17] And so the first thing I'd say is the clinician who is overly organized and prepared.

[00:04:28] Now, organization comes second to preparation.

[00:04:30] That's just a little different.

[00:04:32] Now you can be organized but you can lack the preparation.

[00:04:35] And I think the people who are on Saturday and Sunday reviewing what lies ahead for the week,

[00:04:43] how many clients they have ahead of time, how many patients,

[00:04:46] who's going to be in strength conditioning, how is your work flow going to go from,

[00:04:51] you know, a physical therapy appointment to a post-surgical to a strength conditioning

[00:04:56] to a youth athlete in the evening for performance training.

[00:04:58] Whatever that is, you're going to find your schedule in there.

[00:05:01] But if you were showing up to work and you're super organized when you get to work,

[00:05:06] but you did no preparation, you're going to get punched in the face.

[00:05:08] And what I mean by that is I think there's a lot of people out there who have in PT school

[00:05:13] and in their current situation gotten away with just being organized.

[00:05:17] The people who are obsessive about being prepared,

[00:05:19] overly prepared and thinking about what happens when this patient number one comes in ten minutes

[00:05:26] late, what am I going to do with that scenario?

[00:05:28] Will they take a little bit more on the subject if someone extend that a little bit down

[00:05:32] or I'm going to tighten that down a little bit,

[00:05:34] I'm going to get to their correctives a little quicker

[00:05:35] or I'm not going to leave 15 minutes for them to ask questions.

[00:05:38] So you're always stacking, you know, what's patient one number two,

[00:05:41] how's that tends to be going to be?

[00:05:43] So at the end of the day, that you've stuck to your times,

[00:05:45] you were efficient with your time and the problem becomes most physical therapist.

[00:05:49] You're givers.

[00:05:50] So you don't mind eating up your lunch.

[00:05:51] You don't mind eating up your eating.

[00:05:53] Well, what's funny is you came in the cash physical therapy because you were overworking

[00:05:57] and insurance.

[00:05:57] You can't say that at some point, you have to be able to be strict with your time

[00:06:01] and being able to give that to others, you know,

[00:06:04] whether that's your partner or your kids or whatever it is,

[00:06:06] you have to be a strictly time.

[00:06:07] So in order to get to the point where you're a successful cash-based physical therapist,

[00:06:13] you really need to not just be organized like you were in PDS school or maybe when you first came out of school.

[00:06:18] Now to someone who is overly prepared who knows exactly what's going to happen in the week.

[00:06:23] Now what you can do is understand there's always going to be flexibility in there,

[00:06:26] but you have to prepare for that.

[00:06:28] So my personal routine is always on Saturday morning.

[00:06:30] I'll look at the schedule Sunday morning as anything changes,

[00:06:33] what did I miss?

[00:06:34] And then on Sunday night, I'm just kind of journaling down

[00:06:36] and I'm just going to go out on my top three priorities for every single day.

[00:06:39] And if I can get those top three priorities done,

[00:06:41] then everything else that's going to come in and cause a tornado or chaos is okay

[00:06:46] because as long as I see these three things with these three patients

[00:06:48] or they get their programs that's going to be the most important.

[00:06:52] So that's number one.

[00:06:54] I think if you that set you up for success.

[00:06:56] The second part is you have to have thick skin in cash-based

[00:07:01] and I say that because very politely, if you are...

[00:07:05] If you have a hard time receiving feedback

[00:07:08] and you don't feel comfortable asking for feedback

[00:07:12] or when you get feedback you take it harsh.

[00:07:16] And I think you have to do that because you're in an industry now

[00:07:19] where patient feedback is so critical.

[00:07:21] Now I'm not talking about insurance based feedback.

[00:07:23] I'm talking about real feedback.

[00:07:26] If patient number three is going to leave you

[00:07:29] and they're going to self-destruct.

[00:07:30] And you see them pushing away.

[00:07:31] They're not doing the homework.

[00:07:32] They're showing up late.

[00:07:34] They don't want to reschedule.

[00:07:35] You see them drifting.

[00:07:37] Being able to have that conversation right there and then and say,

[00:07:41] listen, I see in your late,

[00:07:43] I also see that you've canceled all the appointments.

[00:07:45] I'm concerned about our relationship future going on

[00:07:49] to the next visits and being able to achieve results

[00:07:51] and outcomes that you want.

[00:07:54] Can you share?

[00:07:55] Is there any challenges going on that we can address

[00:07:58] so that you can get to the outcomes that you need?

[00:08:00] Being able to do that and some people fear that feedback

[00:08:03] might be like, well that's easy.

[00:08:05] Yeah but you haven't done it yet.

[00:08:06] You're not asking for those feedback.

[00:08:08] You're not asking for things.

[00:08:09] And now they're probably translates to your partners

[00:08:13] and everything else.

[00:08:14] Maybe or maybe you're willing to do that with your partner

[00:08:16] but you won't do a patient.

[00:08:18] I don't understand what the difference is.

[00:08:19] There are both relationships.

[00:08:21] One is, you know,

[00:08:22] bromance love and everything else.

[00:08:24] The other one is financial relationship with them

[00:08:27] and being able to help them achieve an outcome.

[00:08:30] I don't understand why that's so difficult.

[00:08:32] You would think it'd actually be harder

[00:08:33] to have it with a partner.

[00:08:35] And so in other case,

[00:08:36] I think you have to have thick skin because

[00:08:40] this industry is so tough

[00:08:42] if you're not vulnerable

[00:08:43] if you don't have that thick skin.

[00:08:45] You're not willing to get that feedback

[00:08:47] and not willing to push your own limits.

[00:08:49] And I think I'll use the word soft.

[00:08:51] I think a lot of people become very soft

[00:08:53] because PT's going in a strengths-based industry

[00:08:55] and certain jobs have allowed that.

[00:08:58] And I think if you're not okay,

[00:08:59] you don't have that thick skin.

[00:09:00] You're not willing to provide the feedback to patients

[00:09:03] and then receive that feedback.

[00:09:06] I think you're going to be in trouble

[00:09:07] and you won't last,

[00:09:08] even if you're doing it on a side hustle

[00:09:10] one or two days a week or even grow in the five

[00:09:12] you won't have a busy schedule for a long period of time

[00:09:14] and you'll always find yourself

[00:09:15] dabbling in cash-based

[00:09:18] because if you don't have a steady schedule

[00:09:20] and you always have, you know,

[00:09:22] supplementing that with home health

[00:09:24] or travel work or whatever it is.

[00:09:26] So I think if you're comfort-pared,

[00:09:28] you have that thick skin.

[00:09:29] I think that puts you in a really good scenario

[00:09:31] to be able to be successful.

[00:09:33] And number three is I think you have to be optimistic.

[00:09:35] I think that, you know, people have asked me

[00:09:38] a grown to three clinics, 15 staff

[00:09:40] and being cash-based for over 10 years now

[00:09:42] and people have asked me,

[00:09:44] what has led to your success?

[00:09:45] I think that there's going to always be

[00:09:48] a challenge whether that's COVID

[00:09:50] or employees or staff

[00:09:53] or you know upset patients

[00:09:56] or you know challenging things in your business

[00:09:58] there will always be that.

[00:10:01] And I think that if you're a solo-prinore right now

[00:10:03] and you were on your own,

[00:10:06] that's completely fine,

[00:10:07] maybe grow to a team or maybe you don't want to go to a team

[00:10:09] but there's always going to be challenges

[00:10:11] whether you're a travel person

[00:10:13] and you have a flat tire,

[00:10:15] your phone is not charged

[00:10:16] or nothing's going right that day,

[00:10:19] the world is just going to be crumbling on you.

[00:10:22] And that's okay because at the end of the day

[00:10:24] look at the job you have,

[00:10:25] you get to help people and you can say

[00:10:26] focused on that mission long term

[00:10:28] but a lot of people who dabble

[00:10:30] and cash-based, build therapists

[00:10:31] as a physical therapy,

[00:10:33] they think that this is going to be the outlet.

[00:10:35] This is going to be the thing

[00:10:35] that's going to give them freedom

[00:10:36] but when in reality it's them internally

[00:10:39] that's not happy.

[00:10:41] And so this specialty is not going to give you

[00:10:43] the happiness that you're looking forward to.

[00:10:45] That's within you and that's why I'm saying

[00:10:47] the person who's optimistic

[00:10:48] who's naturally optimistic,

[00:10:49] who is grown to be optimistic

[00:10:50] is when who's going to be successful

[00:10:53] because you've met those physical therapists

[00:10:55] who are, you know, life always happens to them.

[00:10:58] It's always their car.

[00:10:59] It's always they forgot their lunch.

[00:11:00] It's always their partner.

[00:11:01] It's always their things when in reality

[00:11:02] that black cloud just hangs over them

[00:11:04] because internally that's what they hold onto

[00:11:06] that's who they are and they are that black cloud.

[00:11:09] And so if you're that optimistic,

[00:11:11] you know, ray of sunshine,

[00:11:12] you're going to do fantastic.

[00:11:14] So the person who's prepared,

[00:11:15] the person who has that thick skin,

[00:11:17] the one who is optimistic

[00:11:18] is going to succeed.

[00:11:20] So that's number three.

[00:11:21] When you put those together,

[00:11:22] you can see the type of person

[00:11:23] that's going to succeed in this industry.

[00:11:25] So number four is

[00:11:27] the person who has grit

[00:11:29] who has perseverance

[00:11:31] because you will be knocked down multiple times.

[00:11:34] You will have patients who when you,

[00:11:37] you know, adjust them and now they're flared up

[00:11:40] and you can't think,

[00:11:42] well gosh, I can't ever

[00:11:43] flet this person up again.

[00:11:45] Now I have to look back and be

[00:11:46] introspective and figure out,

[00:11:47] okay, what went wrong in this area?

[00:11:49] What was wrong about the criteria

[00:11:50] to adjust that person at that time

[00:11:52] or was it that case or whatever it was?

[00:11:56] All of these things are a little intangible things

[00:11:58] that you have to be able to pick up

[00:12:00] on our constantly thinking about

[00:12:02] what could have done better.

[00:12:03] And so the one who perseveres past that,

[00:12:05] maybe you got kicked out of the gym

[00:12:06] that you were working in,

[00:12:08] maybe you're the home health agency

[00:12:11] that you were doing a side hustle with.

[00:12:12] They are no longer supporting your contract.

[00:12:16] Or whatever may be or your lights were turned off

[00:12:18] or your massage table broke,

[00:12:21] all those things will happen to you.

[00:12:23] It's the person who is willing

[00:12:24] to go that next step and have that perseverance

[00:12:26] because this is their passion

[00:12:28] and they're just following it.

[00:12:30] And that's different from being optimistic.

[00:12:32] Optimistic is the psychological like positivity

[00:12:35] and that's fantastic.

[00:12:36] The grit and perseverance is the action

[00:12:38] and that's most important

[00:12:39] but it all starts with optimism

[00:12:41] and you have to be able to push past that.

[00:12:43] And so some people along this journey,

[00:12:46] they can't get it right?

[00:12:48] They might blame it on the clinical skill.

[00:12:49] They might blame it on their neighborhood.

[00:12:51] They might blame it on the city

[00:12:52] that they're in all there's no good cash pay,

[00:12:54] PT patients in here.

[00:12:55] I can't seem to do it, nobody can do it.

[00:12:58] It's just my neighborhood.

[00:12:59] It's just my area where this gym sucked.

[00:13:01] I didn't like that gym.

[00:13:03] They was bad, bad whatever, bad owner, bad whatever it is

[00:13:06] that you need to do.

[00:13:08] But ultimately that's you.

[00:13:09] If you can't stay prepared and you weren't prepared

[00:13:12] to know that and you couldn't stay optimistic

[00:13:13] and ultimately you couldn't push past that.

[00:13:17] You're in trouble, that's a personal challenge

[00:13:19] and I think that's what happens all over the country

[00:13:23] and world and if you're a physical therapist

[00:13:26] who's struggling with all four of these qualities

[00:13:28] will no wonder why you're not gonna be successful

[00:13:30] in cash, PT or your student or new into this

[00:13:33] and you're like, gosh, I thought it would be just simply

[00:13:35] as well let me just bring clinical skill

[00:13:37] into my cash area.

[00:13:40] Well that's different because you were being spoon fed

[00:13:42] by an insurance company and orthos and referrals

[00:13:45] and insurance companies so you were just being spoon fed.

[00:13:48] Now I mean, you eat what you kill at this point

[00:13:50] so the only thing that is gonna get better

[00:13:53] is not your clinical skills but your ability

[00:13:55] to manage yourself and that's what coming

[00:13:57] with over preparedness, being having thick skin,

[00:14:00] being optimistic and having that grit

[00:14:02] that's an internal, these are internal qualities

[00:14:04] that you need to be able to develop.

[00:14:05] So those four and number five is you have to be creative.

[00:14:10] You have to be able to say, okay, we'll great.

[00:14:13] I'm a low back specialist

[00:14:15] but I'm not having any low back patients

[00:14:16] and you go to a low back pain primary care

[00:14:19] and they're not giving you people

[00:14:20] or you go to a low back surgeon

[00:14:22] and they're not feeding anybody in your like woke up.

[00:14:25] You just have to constantly be creative.

[00:14:27] You go to a gym, you go to a local sports club

[00:14:29] and you're just trying to seek out

[00:14:31] and the person who says what I am with back specialist

[00:14:34] but maybe I need to expand

[00:14:35] and maybe you need to expand your specialty

[00:14:36] and be less-nishy or you're too general

[00:14:39] and you need to come into be a little bit more of a specialist.

[00:14:42] All of these, I can't answer for you

[00:14:44] which one is the best for you

[00:14:45] but I need to tell you right now

[00:14:47] if you're not creative

[00:14:48] and you only wanna hold on to that one dream you've had

[00:14:51] you're not gonna be successful

[00:14:52] if you're so laser focused

[00:14:55] on being the successful human being

[00:14:56] that you planned four years ago

[00:14:58] and you've been through ups and downs

[00:15:00] over the last three years

[00:15:01] and you're trying to figure out,

[00:15:02] this is June number four,

[00:15:04] but you're bank account

[00:15:05] but your success hasn't changed.

[00:15:07] It's time to reflect on that

[00:15:09] and I think that there's a difference between persevering

[00:15:12] and just doing the repeated things

[00:15:15] that aren't working or being creative

[00:15:17] and maybe coming back to the after-word

[00:15:19] to maybe expanding a little bit more on this direction

[00:15:21] and maybe coming back afterwards

[00:15:23] as you get more of a patient load

[00:15:25] and I think that that's natural for everyone

[00:15:27] because I think you have this dream

[00:15:29] and maybe you wanna work with all athletes

[00:15:30] and you had two pro athletes on your schedule

[00:15:33] and everyone else was general population.

[00:15:35] You just can't seem to get those pro athletes.

[00:15:37] I don't think that you obsess about that.

[00:15:39] I think you pivot, you'd be creative

[00:15:41] and you adapt to the current environment

[00:15:42] and then you figure that out as you come back

[00:15:44] but if the one who is not creative

[00:15:46] will stick to that and they will just fail

[00:15:48] and they'll just say, you know, cash wasn't for me

[00:15:50] make up whatever excuses you wanted to.

[00:15:52] I didn't like it anyways.

[00:15:54] There wasn't enough people in mind

[00:15:55] in the neighborhood, nobody wanted to pay.

[00:15:57] Whatever it is, it's economy

[00:15:59] and I think that, you know, I think

[00:16:01] overall these things if you're not creative

[00:16:02] there's no way you're succeed.

[00:16:05] So you have to be prepared.

[00:16:07] You have to have thick skin.

[00:16:08] You have to be optimistic.

[00:16:10] You have to have grit

[00:16:11] and you have to be creative.

[00:16:14] And the last one, you guys are have it

[00:16:17] or you don't and that's people skills

[00:16:18] and I think that I think you can grow a portion of it

[00:16:22] but you know people in your PT school or your class

[00:16:25] or even in your job who, they're just not people people

[00:16:29] and they should be researchers instead.

[00:16:32] And so if they're not a people, please or then a people person

[00:16:35] that might grow into it.

[00:16:37] And so they might be good clinically

[00:16:38] and maybe they've done all the research.

[00:16:40] They know they've done all the see you

[00:16:41] as they have all the credentials

[00:16:43] and they're good technically.

[00:16:45] But the minute that you introduce them

[00:16:46] to somebody their heads down, they can't, you know,

[00:16:48] converse and I'm gonna bring it

[00:16:50] over to cash based physical therapy.

[00:16:52] Believe or not, that cash patient

[00:16:54] isn't gonna pay for the skills.

[00:16:55] They're gonna pay for the energy,

[00:16:57] they're gonna pay for their environment.

[00:16:59] Everything that that person creates

[00:17:00] and that in the reality is if they do initial evaluation

[00:17:03] and they're even getting,

[00:17:04] never getting to attempt the technical skill,

[00:17:07] there's no way that that patient's ever gonna book

[00:17:10] out of plan a care because that PT didn't even get

[00:17:14] the chance to gain buy-in from that patient.

[00:17:18] And so I think you have to have a sound

[00:17:21] understanding of how people make decisions,

[00:17:24] what makes them tick.

[00:17:26] Being able to see their body language,

[00:17:27] their eye contact and really put on a show

[00:17:30] when they're working with patients.

[00:17:32] And if you struggle with that in your student

[00:17:34] or your new grad and you struggle

[00:17:36] to be able to entertain clients or athletes or patients

[00:17:40] and you know, make them feel welcome

[00:17:42] and you're not, it's easy to talk to them.

[00:17:44] If you struggle with being able to talk to patients,

[00:17:48] create an environment,

[00:17:50] tell a joke, warm them up and even the anguish person,

[00:17:53] if you struggle with that,

[00:17:55] none of the other five qualities matter

[00:17:56] because cash, basically there it be

[00:17:58] is about people skills

[00:18:00] and your product, your technical skill,

[00:18:02] is irrelevant when you come inside cash

[00:18:04] until you can get the people thing down first.

[00:18:06] Because there's no way you can get someone to commit

[00:18:08] to three, seven, 12 sessions

[00:18:11] if you don't understand what they want and what they need.

[00:18:14] So here you have it, I would say those are my top qualities

[00:18:18] that I think that if you don't have these

[00:18:21] or you cannot develop these, cash-based physical therapy

[00:18:24] is not for you.

[00:18:26] So before you open another clinic,

[00:18:29] you hire another physical therapist,

[00:18:31] you try and gain another specialty,

[00:18:33] you try and move from one gym to another,

[00:18:35] you try and go from your car, mobile therapy

[00:18:38] into a brick and mortar

[00:18:40] or your third year going into cash

[00:18:43] or your insurance-based PT,

[00:18:45] thinking about going into cash-based physical therapy

[00:18:47] if you don't have these,

[00:18:48] I wouldn't even put the energy in.

[00:18:51] I'm gonna save you headaches,

[00:18:53] heartaches, maybe about $100,000

[00:18:55] by just having you think about this.

[00:18:58] Why?

[00:18:59] There's people out there who want you to succeed.

[00:19:02] Your parents, your friends, colleagues, everyone.

[00:19:04] Yeah, you should go be a cash-based physical therapist

[00:19:07] but they haven't lived that.

[00:19:10] Whether it's even your parents telling you,

[00:19:11] your parents haven't lived as a cash-based physical therapist.

[00:19:14] They just know you're a good human

[00:19:15] with a lot of hard work and effort.

[00:19:17] You're gonna achieve it.

[00:19:18] No, if you don't have these things,

[00:19:21] you better address them fast

[00:19:22] so that you're successful

[00:19:23] because if not those cheerleaders

[00:19:24] who were just telling you you're great

[00:19:26] are gonna be wrong and you need to solve that first.

[00:19:29] And now there's nothing wrong with them supporting you

[00:19:31] but I will tell you it's a false sense of positivity.

[00:19:35] So those are the six qualities

[00:19:36] that you're gonna need to enhance.

[00:19:39] Those are the ones that will be indicative

[00:19:41] of success for cash-based physical therapists

[00:19:44] and I hope you continue to develop those.
