⁉️⁉️Thoracic spine stiff bilaterally in rotation? Limited Overhead Mobility⁉️⁉️
The Thoracic Positional Inhibition Technique is one of my favorites and very comfortable! It's indicated when
🔵thoracic rotation is limited bilaterally, generally moderate to severe stiffness in thoracic spine
🔵high tone in the paraspinals - making it difficult to mobilize or perform thrust manipulation
🔵overhead mobility is limited, especially bilaterally
To perform the technique
🔵pt is prone, place your forearms lightly across their back
🔵apply very light P/A pressure and then lightly squeeze your forearms toward each other, slacking the paraspinals
🔵hold the squeeze and pressure for 30-60 seconds
🔵lightly peel your forearms from the patient's back when you're finished
🔵you can test-retest overhead mobility, thoracic spring testing before and after this technique
Since these patients are typically slow responders, you need to work the new mobility slowly and progressively over several weeks using a wall slide regression. I love to use light theraband or an EDGE Suspension Trainer for this. Many times, this significantly reduces pain during overhead mobility, but doesn't reset the mobility. This at least makes mobility work comfortable so they're more compliant with their program. Hope this helps!
