Does it modulate pain well? Yes! Is it easy for the patient to perform away from the clinic - No! When I use manual traction, it's typically for pain modulation to make a repeated end range loading reset strategy as threat free as possible. I haven't had to sell a patient a traction unit more than a few times a year, and that's only for the cervical spine (check out the Neck Saviour on EDGE Mobility System).
