🔥Restoring Terminal Knee Extension🔥
Terminal Knee Extension is one of the best predictors for returning to function s/p ACL repair in the long term. After looking at that in chronic issues in post op knees, I started to look for loss of TKE in any knee. Particularly one where the lumbar spine, hip, and ankle were all clear. The Easiest Knee Test in the world is passively checking for TKE. If the painful knee has a loss of TKE, post op or not, compared to the uninvolved side, try and restore it with the simple mobilization in the video.
Short Lever Arm Version for Sensitive Knees
🔵one hand externally rotates tibia
🔵the other hand internally rotates femur
🔵hold these rotations as your lightly and slowly move the knee into extension
🔵pressure on/off, pain modulation can be enhanced with a mobility band or 2 (proximal and distal to the patella)
Long Level Arm Progression
🔵one hand above patella performing femoral internal rotation
🔵other hand on distal tibia performing tibial external rotation
🔵SB your trunk toward the patient's head so you move the knee into passive TKE
The patient can easily groove the new mobility with self passive knee extension in sitting - think hamstring stretch only it's not static stretching. Repeat these knee extensions hourly. Restoring TKE often helps with WB loading and closed chain knee flexion ADLs and exercises. Make sure to load it up after the resets! Hope this helps!
