😎Counter Rotation for Acute Neck Pain😎
Even though I don't "believe" in acute facet locks anymore, there are conditions where the neck absolutely feels locked up with pinching/sharp pain and severely limited mobility usually in one plane more than the others. Sometimes, people come "addicted to crack" as I say, but are in so much pain, they can't even handle the set up of a thrust manipulation. This technique is one of my favorites that uses a strong traction to modulate pain and restore mobility.
🔵have the patient lie supine and comfortably, holding onto the table
🔵grasp their occiput and under their mandible, having them clench lightly to avoid biting their tongue
🔵lean your bodyweight backward to provide a strong traction, until you see their ankles passively dorsiflex
🔵maintain the traction as you apply a full rotation away from the pain first
🔵maintain traction until you return to neutral, then repeat again after switching hands toward the side of pain
After this, many times the severe pain and limitation is often greatly improved and they can now tolerate your traditional treatments or better yet, repeated end range loading. Hope this helps!
