A great article by Dr. Tom Michaud on Tendinopathy exercise updates made me change the way I've been prescribing exercise. To restore fluid dynamics, using 70% Max Voluntary Contration under a lengthened position is key. To modulate pain/improve cortical inhibition, used pulsed contractions to a beat or metronome has been found useful. Combining both of these and improving time under tension is key to optimal tendon healing. Lmk what you think! I've been using these concepts for rotator cuff, plantarfasiopathy, and lateral epicondylagia as well with good results. Plus the loading programs are better tolerated.
